MJSH provides reliable, person-centred domiciliary care for adults aged 18 and over, including people aged 65+. We support service users to remain safe, comfortable and independent in their own homes. Whether support is needed following discharge from hospital, because of illness or disability, or as part of an ongoing care arrangement, our experienced team will create a care plan tailored to the individual’s needs, routines, preferences and personal goals.

We understand that every person’s circumstances are different. Before care begins, we complete an assessment to understand what support is required, how frequently visits are needed and whether one or two care workers are necessary.
Our domiciliary care services can include:

Safe discharge from hospital
Returning home from hospital can feel overwhelming, especially when additional support is required.
MJSH works with hospital discharge teams, social workers, GPs, community mental health teams and other healthcare professionals to help individuals return home safely.
We can support discharge planning by:
- Responding promptly to referral enquiries
- Completing an individual care assessment
- Reviewing risks within the home
- Developing a personalised care and support plan
- Liaising with the individual, their family and involved professionals
- Arranging appropriate care visits
- Monitoring progress and responding to changing needs

The cost of care depends on the length and frequency of visits, the time of day and whether one or two care workers are required. We explain all charges before care begins and provide a written care plan and price.
